Immunization Requirements
As a medical student, you are required to obtain certain immunizations. The following is an excerpt from your Student Handbook which you will receive at orientation:
Students must submit an immunization record and medical history report as a part of the matriculation process. Most of the required immunizations are equivalent to those required by other institutions of higher education and are reviewed annually with CDC guidelines. With the exception of the three part Hepatitis B vaccine, students must complete and submit an immunization record and medical history report as a part of the matriculation process. In addition students are required to have annual TB skin testing (or CXR documentation of TB skin test +). Students are required to begin the Hepatitis B series before matriculation and must complete the series by the end of February of the first year. In addition, students are required to have a titer drawn to ensure that they are protected. Failure to complete immunizations in a timely manner may result in disciplinary measures.
The following is a list of immunizations you must provide to the Office of Student Affairs by Monday, July 27, 2009. Students with incomplete immunization records will not be permitted to begin classes on Monday, August 3, 2009. Please use the enclosed medical history form to report your immunizations and health information.
Tetanus/Diphtheria: If older than 19 years, must receive a one-time dose of Tdap. Must have a TD or Tdap booster within the past ten years
Measles (Rubeola or Red Measles): If you were born in or after 1957, you must have either documentation of TWO doses of MMR vaccine on or after your first birthday, OR a titer result adequate to indicate immunity.
Mumps: If you were born in or after 1957, you must have either documentation of a two doses of MMR vaccine, OR evidence of immunity by a titer.
Rubella (German Measles): You must have either documentation of a single dose of rubella or MMR vaccine OR a titer result adequate to indicate immunity.
PPD Tuberculosis: Skin test and reading within the past 12 months. A Chest x-ray is required only if the TB skin test reading is positive. The chest x-ray must have been done within the past 12 months.
Hepatitis B: A series of three immunizations and a Hepatitis B Surface Antibody (anti-HBs) QUANTITATIVE titer. The first immunization must be completed by July 27, 2009. The other two immunizations and the titer must be completed before the end of February 2010. (There must be four weeks between the first and second doses, and there must be at least five months between the second and third doses. Titers should be drawn 1-2 months after the third dose. It is recommended that you begin the series now, so that you may finish the series before the February deadline.)
Varicella (Chickenpox): You must submit laboratory confirmation of the disease OR show proof of varicella immunization (a series of two), OR submit a titer showing proof of immunity.
Influenza: A yearly injection for influenza is STRONGLY recommended as you are at increased risk for contracting it and spreading it to your patients. This should be obtained at the appropriate time of the year.
Please send in your medical history form as soon as it is complete. The sooner you get it to the Office of Student Affairs, the sooner you will be cleared to begin classes. You may choose to turn in individual immunization records so that your physician does not have to compile all of your records onto your medical history form. However, we do need the reverse side of the medical history form completed.
